Annual Health and Medical Record
The Boy Scouts of America recommends that all youth and adult members have annual medical evaluations by a certified and licensed health-care provider. In an effort to provide better care to those who may become ill or injured and to provide youth members and adult leaders a better understanding of their own physical capabilities, the Boy Scouts of America has established minimum standards for providing medical information prior to participating in various activities. Those standards are offered below in one three-part medical form.
The Annual Health and Medical Record has 3 parts:
Parts A and C are required for all events that do not exceed 72 hours, and where the level of activity is to that normally expended at home or at school. Part C also includes the parental informed consent and hold harmless/release agreement.
Part B is required with parts A and B for any event that exceeds 72 hours, a resident camp setting, or when the nature of the activity is strenuous and demanding, such as service projects, work weekends, or high-adventure treks.
ALL forms must be updated annually for everyone (Scouts and Adults).
Over-the-Counter Medicine Form
This form allows the Troop to administer approved medications to your Scout.
HINT: A good strategy is to bring along all the Scout medical forms whenever the Scout gets physicals for school, athletics or other camps, so all forms can all be updated at the same time. Most health care professionals will update your records-if you have been in for a physical within the past year-without you coming in for another physical.
HINT: Immunization records (required for camp) may be requested from school nurses or health care givers, as they make sure these are kept up to date.
HINT: Make several copies of medical forms and immunization records, so you never need to give out an original in case records are lost.
